Rehabilitation

Hales Corners Care Center specializes in short term rehabilitation, providing individualized care to each of our patients based upon their specific needs. Our Subacute Rehabilitation program is staffed by licensed, experienced therapists with knowledge specific to the older adult, who use the latest treatment techniques to maximize your functional abilities while preparing you for your successful return home.

Our program offers Physical Therapy, Occupational Therapy and Speech Therapy with services offered 6 days per week. The Restorative Nursing Program complements your rehab experience in order to maintain your strength and functional abilities.

The therapists are an integral part of our discharge planning process, which begins at admission. As you are advancing through your rehabilitation, the therapists will arrange a home assessment. There they will evaluate you in your own environment to ensure your transition home is as safe as possible. A discharge meeting will be held shortly thereafter to discuss discharge arrangements as well as make any recommendations regarding equipment and/or services you may require in order to make your return home successful.
