Social Services

Our Social Service team consists of three Social Workers, each committed to the pursuit of quality of life for each individual. Upon admission to Hales Corners Care Center, you will be assigned a Social Worker who will assist you and your transition into our facility, whether your stay is for short or long term. We strive to meet the full potential in each individual.

For our short term patients, your Social Worker is responsible for coordinating your discharge plans. Should your physician order continued nursing care and/or therapy services, the Social Worker will arrange these services for your prior to your discharge. She will connect you and/or your family with necessary resources in the community. These resources include, but are not limited to: Home Health Care, Homemaker/Companion Services, Home Delivered Meals, Emergency Support Services and Transportation.

The long term care Social Worker is responsible for assisting you and your family through the transition into long term care. Your social and emotional well-being is our utmost interest. The Social Worker is an integral member of our interdisciplinary team which assesses the resident and develops a plan of care. The Social Worker will encourage you to become involved in the life within the facility.
